This position is for an independent contractor who will assist patients of our partner home health care agencies.

Responsibilities:

Ensures up-to-date knowledge and updated professional field competence.
Makes sure that the provided services and treatments are in compliance with the guidelines of the Physical Therapy licensing and practice act norms.
Cooperates closely with treating doctors or physicians, care coordinators and PTAs within the scope of treatment duration.
Supervises Home Health Aide when required.
Informs and educates the family members or the caregivers about the challenges that they or the patient might face within the recovery process.
Follows the regulations set forth by the Physical Therapy Professional Scope of Practice as well as the rules set forth by the Agency.
Evaluates and records patient’s progress, modifies treatment plans or tries new treatments or techniques if required after the consultation with the physician.
Uses exercises, stretching equipment, different stretching techniques as well as hands-on therapy to manage patient’s Plan of Care, which will result in increasing the patients’ mobility and will alleviate any further pain or injury.
Outlines clearly the goals that the patient must work towards and the expected outcomes of the Plan of Care.
Develops individualized treatment plans for patients based on their needs, thus providing an appropriate and up-to-date Plan of Care that will be based on medical diagnosis and patient’s medical records.
Reestablishes patient’s functional mobility, averts pain by implementing different treatment techniques based on medical prescriptions.
Evaluates and reviews patient’s medical histories and diagnoses patients by observing their movements and listening to their concerns.
Assesses and diagnoses patients who have been suffering from physical disability which was resulted from an injury, accident, surgery or functional mobility problems due to diminishing of the physical strength, frailness or balancing issues at the comfort of patient’s home.

Qualifications:

PT must operate under one’s own business entity such as an LLC, S-Corp, or Professional corporation
Valid Driver’s License and State required Auto Insurance
BLS Certification
Valid California Physical Therapy License
Minimum of one-year experience.
Degree in Physical Therapy from an approved program pursuant to Medicare guidelines

We are currently looking for specialists in the areas of: Los Angeles

This position is for an independent contractor who will assist patients of our partner home health care agencies.

Responsibilities:

Ensures up-to-date knowledge and updated professional field competence.
Makes sure that the provided services and treatments are in compliance with the guidelines of the Physical Therapy Assistant licensing and practice act norms.
Ensures that other medical specialists have the necessary information for an accurate diagnosis.
Completes and performs the necessary documentation for supporting the delivery of physical therapy services as well as obtains accurate information and selected data collection for the medical records.
Records and submits medical and progress reports that are based on treatment evaluations and accomplishments that should indicate the reaction or any changes in patient’s condition. The reports should be submitted the Physical Therapist or the Care Coordinators.
Instructs and counsels patients effectively in exercises.
Implements direct physical therapy sessions in accordance with the Plan of Care and under the supervision of the Physical Therapist.

Qualifications:

BLS Certification
Valid California Physical Therapy Assistant License
Graduate of an accredited Physical Therapy Assistant program with either an associate degree in science or applied science that is an approved program by American Physical Therapy Association, Inc

Administers patient care in compliance with agency policy, law and regulation, and according to the guidelines provided by the Occupational Therapy Practice Act, Occupational Therapy policies and procedures and scope of care.
Evaluates patient’s condition regarding physical and basic mental health as well as establishes goals that Occupational therapy will treat and adjusts programs based on patient’s needs and skills.
Establishes physical rehabilitation programs to help patients regain lost skills and prepare them to return to normal life.
Develops physical rehabilitation programs to help patients regain lost fine motor skills.
Advises on what equipment would suit the patient for their everyday activities and helps them utilize them.
Assesses the patient’s home and work environments and makes necessary adjustments if needed.
Implements physical and psychological assessments of the patient and indicates treatment plan using a variety of methods.

Qualifications:

Home Care Experience (Preferred)
Occupational Therapist must have one’s own business entity such as an LLC, S-Corp, or Professional corporation
Minimum one-year experience
Valid California Physical Therapy Assistant License
BLS certification
Licensed/Registered by the Occupational Therapist Board of California
Master’s Degree in Occupational Therapy

Administers patient care pursuant to the terms proscribed by the Speech-Language Pathology Practice Act, Speech-Language Pathology policies and procedures and scope of care.
Counsels and educates patients, families and care givers. Cooperates with them closely to have better understanding of the treatment needed.
Initiates and implements therapeutic interventions, evaluate the treatment’s efficacy, and modifies prescribed therapeutic interventions if needed.
Assesses the patient’s progress and prepare detailed reports.
Observes and communicates with family members, physicians and teachers to design effective treatment plan and develop and individual speech and language programs based on the assessment results.

Qualifications:

Home Care Experience (Preferred)
SLP must have their own business entity such as an LLC, S-Corp, or Professional corporation
Minimum 1-year experience in providing Speech and Language Services
Valid California Physical Therapy Assistant License
BLS certification
Licensed/Registered by the Speech-Language Pathology and Audiology Board of California
